  • We consider applicants with secondary substance use disorder diagnoses. We request a minimum 30 days of sobriety from all substances, completion of SUD treatment is preferred if there has been a SUD within the last year.
  • Andrew Residence is not a skilled nursing facility; applicants must generally be independent with ADLs.

The Pre-Admission Process:

We will request a list of contacts, that will include the applicant’s psychiatric provider, physician, guardian information and county case manager. Other information requested would be a Diagnostic Assessment or History and Physical and any recent progress notes if applicable.

Once the information is reviewed, and the applicant meets initial screening criteria, a preadmission interview will be scheduled with one of our social workers.

Following the interview, the applicant or referral contact will be contacted by the Admission Coordinator on acceptance typically within 1 business day.

The Admission Process:

If accepted for admission, please provide:

* Completed forms provided to the applicant at the preadmission interview:

1. 1. The Andrew Residence Medical History, Physical Examination, and Admission Orders form (within 5 calendar days prior to admission), form must have MD or DO signatures
   2. The Andrew Residence Psychiatric Assessment and Admission Orders form and a follow up psychiatric provider appointment scheduled
   3. Mantoux or TB Gold Test and if required chest x-ray (within 90 days)

* Civil Commitment/PD/Jarvis, guardianship, probation, or any other legal papers, if applicable
